    Session Description: 
    Do you ensure that the care needs of your school-based clinic students' are coordinated? Does your health center provide the same types of services at your school-based clinics, just like your primary care sites? The NCQA SBMH Recognition program is a hybrid model program that evaluates how school-based health centers perform as a medical home, provide episodic care for students with urgent or emergent needs and act as a collaborative care center for students and the community served. School-based health centers seeking recognition demonstrate they meet NCQA PCMH requirements and complete the school-based health center subset of requirements in the same evaluation. HRSA is not covering the cost of NCQA SBMH recognition. They will cover PCMH recognition at a school based health center.  Let's start you on the path to become a recognized medical home at your school-based centers.

    About the Speaker:  
    Dawn Gentsch is a population health improvement coach. She has supported, and learned from, approximately 50 primary care practices across the United States in work as a practice facilitator, including serving as a patient centered medical home coach. She is an NCQA PCMH certified content expert and has 15+ years working with primary care practices. She began providing PCMH support for KPCA members in early 2013 and continues to do so today.
     
    Dawn is a health educator who has worked in public health, primary care, academia, and with various non-profit organizations. Focus areas include practice transformation, systems change, maternal and child health and innovative population health strategies. She has managed numerous federal health focused grants and serves as an external health promotion evaluator. Dawn lives in Iowa and supports practices remotely with an occasional site visit as well as on site trainings.
