Registration fee includes lunch and refreshments.

PLEASE NOTE: Because this conference is funded primarily through conference registration fees, all persons must be registered to attend. No refunds will be made for cancellations received after December 1, 2008. However, a substitute may attend in the place of an unavailable registrant. All registrations and payments are due by Monday, December 1, 2008.

Important changes: All UDS data entry will be through the EHB; grantees will not receive a software disc from the UDS contractor as in the past. In light of the new UDS reporting requirements, it is especially important for key health center staff responsible for the UDS Report to attend the trainings.
