| Description
- Services Park DuValle Community Health Center, Inc. is a Section 330 funded federally qualified health center serving predominantly Jefferson and Spencer Counties of Kentucky. Residents of counties abutting Jefferson and Spencer Counties (Anderson, Bullitt, Henry, Nelson and Shelby) may also be served at any of the Park DuValle CHC four locations. All sites are licensed Primary Care Centers and are accredited by the Joint Commission of Accreditation of Healthcare Organizations (JCAHO). The main site for Park DuValle Community Health Center is located at 3015 Wilson Avenue. Services offered at the main site include: preventive health services, primary adult and pediatric medical care, women’s health, family planning, dermatology, ophthalmology, primary dental care including dental hygiene, social services, nutrition counseling, immunizations, pharmacy, x-ray, laboratory, Women, Infants, and Children’s (WIC) Program, and transportation services. Mammography is offered one week per month. Free health education, disease management, smoking cessation, life skills and wellness classes are provided for patients and members of the broader community. Three satellite sites offer preventive and primary health and medical care services and pharmacy services. |
