| Description
- Services Kentucky Mountain Health Alliance, Inc. and Harlan Countians for a Healthy Community, Inc. work together to coordinate a 330(h) Healthcare for the Homeless project. The project provides primary health care and supportive services to the homeless or at risk for homelessness individuals in Perry and Harlan County, Kentucky. The project is also supported by medical and dental providers who volunteer to see homeless patients either on site or in their office and by other contributors who help purchase and access medications, medical supplies and diagnostic services. KMHA also contracts with vendors who provide discounted medical care, diagnostic and other services for our patients. In Perry County services are provided at Little Flower Clinic. In Harlan County the services are provided at Clover Fork Clinic. |
