Description - Services Established in 2003, Juniper Health, Inc. (JHI) is a private, not-for-profit Community Corporation that provides quality primary medical care and a variety of other health services to residents of Breathitt, Lee and Wolfe counties in southeastern Kentucky. The Mission of Juniper Health, Inc. is to enhance the health and quality of life of individuals and families living within the communities it serves by providing quality health care within a safe, supportive and welcoming environment without regard to the patient’s ability to pay. Its Vision is to be the area’s source for affordable medical care, be recognized as leader in the delivery of community health services, provide individuals and families with the education and support to achieve health living, and become the area’s provider of choice for primary health care services. In their first full-year of operation in 2006, JHI's two health centers combined to provide comprehensive medical care to some 4,500 individual residents of its three-county Service Area, and experienced nearly 13,000 patient encounters. All payors are accepted. To apply for the sliding fee option, patients should bring income documentation: Previous years income (W-2 form) or tax return Current paycheck stubs Letter form the employer must be written on company letterhead If self-employed, a Form 1040 tax return from previous calendar year State income – letter from social worker, copy of award letter from the Social Security Administration, check stub or letter with details of pension benefits.
