Receives patients and directs them to appropriate area.
Schedules patient appointments with social workers, psychiatrists, psychology students, case managers, patient advocates, and volunteers.
Writes vouchers against the Medicine Fund to pay for medicines purchased through FHC pharmacy.
Makes exceptions to Medicine Fund guidelines as appropriate for a patient's circumstances.
Triages and interviews patients on a walk-in basis to determine for which medicine assistance programs they are eligible and assists them in obtaining medicines.
Consults with area community ministries to coordinate medicine assistance.
Answers telephones and directs calls to appropriate personnel.
Initiates medical charts for patient appointments and walk-ins.
Processes applications for Kentucky Physician's Care Program (KPC).
Prepares monthly reports for the Medicine Fund, Medicine Assist applications, inappropriate referrals, and other reports as needed.
Processes and documents referrals by primary care providers.
Notifies patients who failed to keep appointments.
POSITION QUALIFICATIONS:
High school graduation or its equivalent.
Two years of clinical/clerical experience in a behavioral health/mental health setting.
An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.

Conducts initial screening visits of 30 minutes or less.
Evaluates level of functioning and determines level of risk.
Determines if referral to another agency is appropriate.
Assists with diagnosis and behavioral management.
Makes treatment/referral/follow-up recommendations to PCP.
Assists patient with compliance of planned interventions.
Conducts group classes to promote patient skill building or education.
Provides information on disease, proposed interventions, coping with side effects of medications.
Participates in conjoint consultation with PCP and Behavioral Health (BH) staff members.
Travels to satellite clinics to provide services to PCPs and patients.
Participates in program planning and development.
Provides in-service training, education to PCPs and BH staff.
Assists with preparation of statistical and other reports.
POSITION QUALIFICATIONS:
Master's Degree in Social Work.
Possession of a Licensed Clinical Social Worker license from the State of Kentucky.
Must possess a valid driver's license.
One year of recent (within past 3 years) experience in providing one-on-one and group clinical social work services which must have included the use of a personal computer involving internet and word processing software.
Verification of education, Clinical Social Worker License and valid driver's license must be provided at time of application.
SPECIAL REQUIREMENTS:
Must provide own transportation and auto insurance.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS:
Lifts and carries items weighing up to 10 lbs.
Communicates with individuals face-to-face and over the telephone.

The successful applicant will participate in a full range of undergraduate and graduate teaching programs as well as all facets of patient care including out-patient assessment, inpatient care, major operative procedures, and outreach activities. The successful candidate will join our busy rural multidisciplinary team with a large referral base.

Lexington Nurse Practitioner at Bluegrass Community Health Center Easter Kentucky University is accepting applications for a Lexington-based Bluegrass Community Health Center Nurse Practitioner to provide direct primary health care for adults with both acute and chronic conditions, within the scope of established medical practice protocols and in collaboration with the medical director.

Minimum requirements include:
  • A Master's degree from a regionally accredited or internationally recognized institution;
  • A license to practice as a Nurse Practitioner in Kentucky
  • Two years of community-based public health experience.
