| Description
- Services Family Health Centers, Inc., (FHC), a Federally Qualified Health Center, is the hub of an organized network of primary care centers in Louisville and Jefferson County that are committed to meeting the needs of the medically underserved. Family Health Center Portland, the main site, is located in Louisville’s West End and serves as the service delivery hub, with four satellite facilities extending primary care and preventive care throughout the community. The five satellites are as follows: FHC-East Broadway, FHC-Iroquois, FHC-Southwest, FHC-Fairdale and FHC-Phoenix. FHC-Phoenix provides Primary Health Care for the area's homeless patients. Family Health Centers, Inc. offers a broad range of primary care services, including Medical, Dental, Behavioral Health, Laboratory, Radiology, Pharmacy, Social Services, Health Education and Public Health Services. Family Health Centers, Inc. is accredited by the Joint Commission on Accreditation of Health Care Organizations. |
